Covid vaccine children 6 months under 3 years

From 6 months to 3 years prescription is required.

If you believe you are sick, please wear a face mask going to the pharmacy

1. Make sure to download and read the VACCINE INFORMATION FACT SHEET
2. Bring a copy of the child’s birth certificate.
3. A parent or legal guardian must accompany the child to be vaccinated.
4. Fill, sign and bring a copy of the consent form for minors attached
